    After several years of investigation, the only genuine cognitohazard I managed to find was a short phrase that gets automatically filtered from every communication platform I could find. It is impossible to post this phrase anywhere or otherwise communicate it digitally. Any post or string that contains all the words in any order vanishes. If you post any one of the words and attempt to follow it with another post containing another, both posts vanish. The filtering seems to take place at a very deep level - a simple PHPBB board that I made specifically for this purpose still filtered it, even though I was posting as an administrator. I also tried writing it in a text document and screenshotting it, and even writing it by hand and taking photographs at varying angles, but some sort of word recognition must be in operation because none of those pictures could be successfully uploaded anywhere. The phrase cannot be searched on any search engine that I tried, and I suspect that the filtering is actually being done by some routine running on my hardware. The oldest computer I have had access to dates from 2010 and behaves in the same way. I was made aware of this phrase by some very cryptic hints and riddles that I can't accurately recall and can no longer find online. The best I can provide is this:

    I agree to some extent, some information has the ability to negatively effect people's minds. At the most basic level, let's say we lie to a group of people that a single person must die to save the group. This false information negatively effects individuals psychologically and can lead to violent action.

    All types of information can be potentially harmful from how to make a bomb or video recordings of humans being tortured.

    As for adverse physical effects certain flashing images can cause seizures and jump scares can cause heart attacks. Both of these effects are directly harming the person who views them.

    As for a living entity, it might be worth while diving into mental constructs. If a fictional entity can gain enough belief from an individual it is possible for them to be translated into dream's and even hallucinations for the mentally unwell.
